Home Visit
We give families the opportunity to get to know us better during the face-to-face visit. One of our Client Services Managers will evaluate your loved one’s needs, as well as those of your family. We will set goals for care, create a schedule to match your needs, and discuss costs and funding options.
Finding the Perfect Caregiver
We want your home care experience to be successful. A key to that success is a warm and trusting relationship between your loved one and their caregiver. We consider everything we’ve learned about your loved one to assign someone who best matches your loved one’s personality and needs. If we don’t get it right the first time, we’ll keep trying until we do.
Nurse Evaluation
Once you’ve chosen Elder Independence Home Care to care for your loved one, an experienced nurse will visit to perform an assessment of your loved one’s physical, mental and social needs. The nurse will use the assessment to develop a personalized plan of care that will be followed by the caregiver every visit. One of our nurses will routinely provide reassessments if there’s been a change in your loved one’s condition, if your loved one has recently been released from a hospital or rehab, or to check in to see if our caregivers are meeting your expectations.
Round-the-Clock Assistance
If you or your loved one has an emergency, we are here to help. A member of Elder Independence’s Management Team is on call 24 hours a day, seven days a week (including holidays), to assist with any situation that may arise. Even if it’s something we can’t solve on our own, we can work through it with you so you don’t have to face it alone and help point you in the right direction.
Exceptional Care
Our services are available 24 hours a day, seven days a week. With a staff of more than 150 caregivers, we will work hard to cover any shift you may request. We provide ongoing education for our caregivers so they are knowledgeable in all areas of home care.
